Home-bound Individuals
- If you or someone you know is an individual that cannot leave their home and would like to be vaccinated, please call our vaccine hotline at 517-264-4501 to get scheduled. The Bivalent Booster vaccine is also now available for homebound individuals.
COVID-19 Vaccinations for Individuals with Disabilities
- The Disability Information and Access Line (DIAL) is available to help people with disabilities, including Deaf persons, get vaccinated.
- Staff can help to:
- Find vaccine locations
- Assist with making appointments
- Connect callers to local services - such as accessible transportation - to overcome barriers to vaccination.

Vaccination Cards
If you or your child were vaccinated outside of Michigan, you can upload a copy of your vaccination card and we can enter it into MCIR, the Michigan Care Improvement Registry. A google account or gmail address is required for this process. Complete Vaccination Form
