Letter to Parents (Spanish)
If you want your child vaccinated at school, forms were sent home from school but can also be downloaded here. Both the consent form and the H1N1 scan form need to be completed. Please print clearly in CAPITAL LETTERS with black ink. Insurance information is NOT required on the form. There is NO COST for this vaccine. Children without the completed forms will NOT be vaccinated. If you do not wish to have your child vaccinated, please sign the REFUSAL SECTION of the consent form and return the completed form to your child's school. ***NEW: Currently we have the H1N1 Vaccine Nasal Spray for school clinics. If you would like your child to receive the nasal spray vaccine,
please read the Vaccine Information Statement on Nasal Spray (below), and
print this form and return it to your school along with the consent form and scan form.
